If you would like help applying for the Healthy Michigan Plan, please contact Harbor Beach Community Hospital’s Community Connections Program at 989 550 0972. This program covers those who are:

At or below 133% of the federal poverty level under the modified adjusted gross income methodology

Not enrolled or do not qualify for Medicare

Not enrolled or do not quality for other Medicaid programs

Not pregnant at the time of application

Hospital Charges

In 2019, all hospitals were required to post their current charges on the internet. This requirement was expanded in 2021 to include average allowable amounts for those plans with which the hospital contracts. These updated lists for Harbor Beach Community Hospital are found below.

Please refer to the plan document provided by your insurance company or call the customer service number on your insurance card for information on your out of pocket cost. Prior to calling you should obtain the CPT and the ICD 10 codes from your provider’s office for the service you are inquiring about.
